Remote Work for Doctors Outside the US: Eligibility, Pay and Taxes (2026)
Many remote healthcare roles are open to doctors and other clinicians outside the United States, but not all of them. This guide uses SOJI's open roles on 14 October 2026 to explain eligibility, how pay works from abroad and what to check about tax.
Which roles are open outside the US
Of the 59 open healthcare, medicine and mental-health roles, 32 are open worldwide and 23 are limited to the United States. The split depends on the specialty:
- Bilingual psychiatrists and psychologists: all 8 are open worldwide, but they ask for a second language such as Japanese, Vietnamese, Cantonese or Georgian.
- Hospitalists: 8 of 12 are open worldwide.
- Physicians: only 2 of 10 are open worldwide, and most ask for a US licence.
- Nurses: 2 of 5 are open worldwide. Medical coding: none of the 2 roles.
"Worldwide" means the listing accepts applicants from any country, but the qualification still applies. Each listing states where applicants can be located, so read that line first. See the full picture in remote healthcare jobs: what is open and what it pays.
Licence and language
Some roles ask for a licence in a specific country, and others accept a recognised medical degree and years of practice. Language can be an advantage: several of the best-paid mental-health roles exist because the project needs a native or near-native speaker. State your languages clearly when you apply.
Getting paid from abroad
Platforms usually ask contractors for identity and payment details, and they pay in US dollars by bank transfer or a payment service. Check which payment options reach your country before you start, and expect exchange rates and transfer fees to reduce what arrives. More in how to get paid by AI training platforms from abroad.
Taxes
This is general information, not tax advice. As an independent contractor you usually declare the income in your country of residence as self-employed income, and no employer withholds tax for you. What you keep depends on your country's rules and regime. Try the freelance tax calculator for Portugal, Brazil, Spain, Mexico, India or Nigeria, and keep every invoice and payment record. A local accountant can tell you which regime applies to you.
Before you start
- Check the listing's location, licence and language lines.
- Check your employment contract and professional rules on outside work.
- Never use real patient information in a test or task.
- Work out your net pay with the calculator, not from the hourly rate alone.
- Keep invoices, payment receipts and the platform's contract.
Frequently asked questions
Do I need a US medical licence?
It depends on the listing. Physician roles often ask for a US licence, while many mental-health and hospitalist roles are open worldwide. Read each listing.
Can I be paid in my local currency?
Listings show pay in US dollars. The payment options depend on the platform and your country, and conversion costs apply.
Will I pay tax twice?
That depends on your country's rules and on any tax treaty between countries. Ask an accountant who knows your situation.
Can I do this alongside my hospital job?
Many people do, because the work is remote and part-time, but check your employment contract and professional rules first.
Limitations
Counts and pay come from SOJI's listings on 14 October 2026 and change as roles open and close. Applying does not guarantee a project. This is general information, not tax, legal or professional advice.
SOJI is an independent job board and is not affiliated with Mercor or Micro1. If you apply through our referral links and are hired, SOJI may earn a commission, at no cost to you, and this does not affect which jobs we publish. More on the about page.
